Please complete and submit this consent form within 48 hours of our first spiritual direction session. Thank you! — Melissa Auten Spiritual Direction with Melissa Auten Consent Form Spiritual Direction, in person, in writing, by phone, or by video, is a process entered into voluntarily by the client (or directee). It is time set aside to reflect with intention on life experiences in relationship with God. The director fills the role of companion on the journey through listening, reflecting, prayer, and guided discernment. The individual in spiritual direction takes responsibility for the content and flow of the material shared. Spiritual direction is not therapy, counseling, or pastoral care. I understand and acknowledge each of the following ethical responsibilities and limitations of my Spiritual Director as listed below. 1) My Spiritual Director reserves the right to carefully choose with whom to work. She assesses the appropriateness and the maximum number of persons whom she can guide, while still maintaining the ability to attend to and discern the optimal way to provide guidance. 2) My Spiritual Director understands the psychological issues that may arise within the context of the guiding relationship when two souls meet and share intimately about matters of the spirit. She knows how to deal with these issues by addressing issues of transference and counter-transference. Despite the close nature of the Directee-Director relationship, this is not a two-way relationship. It is not the Directee’s place to take care of the Director’s feelings and it is not the Director’s place to discuss her own issues. 3) My Spiritual Director maintains respect for my freedom to find my own spiritual path and she helps to guide me in this regard. My Spiritual Director encourages me to develop a balance between contemplation and action in my spiritual growth. Telephone and email sessions with my Spiritual Director are characterized by privacy, acceptance, and confidentiality. Occasionally, my Spiritual Director may consult with pastors regarding needs for other support services for directees, but only if they are members of the same congregation. Under such circumstances, the Director is mandated only to disclose relevant information in order to provide the appropriate help. 4) My Spiritual Director cannot be held responsible for providing services to me in the event of a crisis or emergency situation. I may need to find a local therapist if such a situation arises. 5) Spiritual Direction will take place in person, by phone, Zoom, or email as agreed between director and directee. Please cancel appointments 24 hours in advance. 6) There are unique limitations to engaging in spiritual direction online, Zoom or by telephone as opposed to in person. I recognize and accept these limitations. 7) My Spiritual Director is accountable to a supervisor and is in consultation to maintain the integrity of the practice. Session material may be brought to supervision or consultation. Confidentiality will be maintained. 8) Spiritual Direction is normally paid for by an honorarium fee to the director. No one will be turned away who desires direction due to a lack of resources. As a general suggestion, consider paying your director what you earn per hour, within reason. Fee may be paid by personal check, cash, Venmo or PayPal. I acknowledge having received and read Mosaic Heart Ministry Client/Directee Consent Form and accept the terms as written.
